Initial Intake Session

At this appointment, EIMOC staff will talk with you about your goals, challenges, and motivation for exercise. EIMOC staff will provide more information about the program, and you’ll be asked to complete paperwork, including a medical history form and informed consent. You will be asked to complete an online questionnaire that will be used to pair you with a trainer.
